Fellow Logs

Just a reminder to each of you on your logs:

You must submit your logs to the Support Staff 10 days prior to your leaving the program for them to be certified. You will not be allowed to graduate without this certification.

Logs must include the following:

  1. Cath Lab
  2. TEE/Hand’s on Echo
  3. Nuclear
  4. EPS
  5. Also, we need an estimated number of the EKG’s you have read throughout the training program.

Please make sure your logs are documented with medical record number for the patient, date of birth of the patient, procedure, location (hospital), date of service and the attendings name. Failure to provide all of the necessary information could affect your graduation date and time. Please be sure to have these logs prepared far in advance of your graduation date.
